Join the Collective!
All patients wishing to become a member of CCC are required to be qualified medical cannabis patient residing in the State of California.
Patients must fill out and return the completed Membership Application along with your original Physicians Recommendation via hand delivery to your assigned caregiver. You must also show proof of a valid California's drivers license or identification card. All required documents must be presented before your first visit, no exceptions.
After receiving your application, your patient status must be verified prior to your first caregiver visit. Membership will not be approved without verbal verification of your patient standing by your Physician's Office. Please note your membership may be denied for any reason.
If accepted, there will be a one time non-refundable membership fee of $15 (fifteen dollars).
Cannivan is a CA registered Mutual Benefit Corporation, operating on a not-for-profit basis in compliance with all State and County laws and regulations, within the CA Attorney General Guidelines for the lawful operation of medical marijuana collectives.
*Medical Marijuana remains unrecognized and illegal under Federal law.
